Use our repair request form to describe your device and the fix it needs. You can pick mobile service, where we travel to a location of your choosing, or store service by appointment. Just include your name, phone number and preferred contact method so we can get back to you about your request.
Which device should I choose on the form?
Select your iPhone, iPad or iPod model from the Device drop-down. Not sure which model you have? Follow the Identify Your Device link next to the drop-down before submitting.
Mobile Service or Store Service — which fits me?
Pick Mobile Service if you'd like us to meet you at a location you select; the form will ask for the meeting address, including street, city and zip. Pick Store Service if you'd rather come to the shop by appointment.
How do Glass Replacement and Glass & LCD Replacement differ?
These are the two screen repair choices in the Repair drop-down. If you're unsure which one your device needs, choose “I don’t know” and describe your device's current condition in the text box. The form also links to a Glass Only vs. Glass + LCD explanation on the FAQ page.
What goes in the description box?
The description box is required when you choose “Other” for the repair or aren't sure which repair is needed. Describe your device's current condition — for example, a cracked screen, a battery that won't hold a charge, or a damaged camera or charge port.
How will you reach me about my request?
The form asks for your name, phone number and preferred contact method (Call or Text). Provide the number where you'd like to be reached so we can follow up on your repair request.
